The characters' aesthetic similarities were emphasized by the choice of voice actress, Kotono Mitsuishi, the original performer of both roles some staff members, such as Jun'ichi Satō, had already worked for the animated version of Sailor Moon. Sadamoto accordingly drew her taking his cue from Usagi's character design, and picking up her hair. To outline her personality, he took inspiration from Usagi Tsukino, the 14-year-old protagonist of the Pretty Guardian Sailor Moon series, thinking of Misato as a more adult Usagi. As with other Neon Genesis Evangelion characters, for her name the director was inspired by a Japanese Imperial Navy ship, the Japanese aircraft carrier Katsuragi, and Misato Izumi, a female character from Minako Narita's Aitsu manga.
At the request of Yoshiyuki Sadamoto, character designer of the series, the initial scenario was changed and the protagonist became a male, Shinji Ikari some ideas for the older sister flowed instead into what would become the character of Misato. Hideaki Anno initially proposed to include a female protagonist for Neon Genesis Evangelion, a girl similar to Asuka Langley Soryu the young woman was to be accompanied by a female soldier, a kind of "big sister" similar to Kōichirō Ōta's character from his previous work, GunBuster.
