The solution is to open charmap.exe, then Group by: Unicode Subrange, then click General Punctuation, and then select and copy the double exclamation mark character (n.b., not the single exclamation mark twice, but the double exclamation mark) and then open your XML file in notepad and do a search for the double exclamation mark characters. I had the same issue as steveglines I imputted many segments only to find that they were missing on re-loading.
